Garage Door Quote Form — 13 fields
Copy fields to use in your own formYour name
e.g. John Smith
Why: Personalises your follow-up and makes the lead feel like a relationship, not a transaction.
Phone number
e.g. 0412 345 678
Why: Primary follow-up channel — critical for high-urgency leads that need a same-day call.
Email address
e.g. john@email.com
Why: For sending your quote and any follow-up documentation.
Suburb and state
e.g. Ringwood VIC 3134
Why: Confirms you service the area and lets you estimate travel time to site.
Current door type
e.g. Tilt door — single garage, or Sectional panel lift
Why: Determines the product category, spring system requirements and panel compatibility.
Opening width and height
e.g. 4800mm wide × 2100mm high (rough measure is fine)
Why: The single most important measurement for pricing. Even a rough size narrows the quote significantly.
Are you replacing an existing door or installing new?
e.g. Replacing old tilt door from the 1980s
Why: Replacement jobs require removal, disposal and potentially new tracks — different scope and cost.
Do you want a motorised door?
e.g. Yes — we'd like remote access and keypad entry
Why: Motor vs manual is the biggest single pricing variable after door size.
Photo of the full door from the driveway
Upload from your phone
Why: Shows the door width, style, condition and surrounding clearances in a single shot.
Photo of the motor or drive mechanism (if existing)
Upload from your phone
Why: Helps identify the current operator brand and model for compatibility assessment.
Photo inside the garage showing the ceiling and overhead space
Upload from your phone
Why: Confirms headroom clearance — critical for sectional door conversion or motor installation.
Approximate budget range
e.g. $2,000 – $4,000 or no specific budget yet
Why: Matches the product specification (standard steel vs Colorbond vs timber-look) to the customer's expectations.
When do you need this completed?
e.g. Urgent — door not closing properly, or Within 6 weeks is fine
Why: Helps you prioritise callbacks and plan installation crew availability.
How to use this template
Copy the fields above and add them to your existing website contact form, or use them as a starting point for a dedicated quote intake page. The order of the fields matters — start with contact details, then move to job-specific information, then photos and commercial details last.
If you use a multi-step form (one question per page), completion rates improve significantly. Customers are less likely to abandon when they see one question at a time rather than a long single-page form.
For best results, include a brief explanation next to each field — especially dimensions and photos. Customers who understand why you need the information are more likely to provide it accurately.

FAQ
What's the most important field in a garage door quote form?▼
Opening dimensions (width and height) and a photo of the existing door. Together these two pieces of information let most experienced installers give a preliminary price range without a site visit.
Should I ask for dimensions in my quote form?▼
Yes — but frame it gently. Say 'even an approximate measurement helps' and explain that a standard tape measure from inside the garage opening is sufficient. Most customers will provide it.
Do I need photos in a garage door quote form?▼
Strongly recommended. A single exterior photo of the existing door shows the style, size, wall clearance and condition — saving multiple follow-up questions. Ask for one and most customers will provide it immediately from their phone.
